Perhaps you’ve moved address or got a new mobile number? We need to know changes like these, so that we can follow your health and mobile usage over the long-term.
You can provide up-to-date details as part of the 2016 follow-up questionnaire (click ‘Start Questionnaire’ on the home page or top menu bar to begin).

It will take 3 minutes to update us – we ask you to check your personal details are correct and up to date, followed by 6 short questions about your current mobile phone(s). You will need your Unique ID and Password to log-in.
