COSMOS Study Prize Draw Terms and Conditions

1. By completing the COSMOS online survey which relates to your mobile phone usage, health and lifestyle (the Survey), you will be automatically entered into the prize draw to win one of 100 £25 high-street vouchers.  
2. The prize draw is run by Imperial College of Science, Technology and Medicine.
3. The prize is non-transferable and there is no cash alternative.
4. The prize draw is open to residents of UK aged 18 or over who have been invited to complete and who fully complete, the Survey.
5. The closing date for eligible entries to the prize draw is 30 June 2010.
6. Only one entry is allowed per person.
7. The winners will be drawn on 12 July 2010 and notified in writing by email by the 16 July 2010.
8. No responsibility will be taken for entries which, for technical or circumstances beyond the reasonable control of Imperial College of Science, Technology and Medicine, are not entered into the prize draw.
9. The winners of the prizes will be the first 100 names drawn at random from all eligible entries.
10. The names of the prize winners will be made available to anyone sending an email to ukcosmos@imperial.ac.uk within 14 days of the prize draw.  No other correspondence will be entered into.
11. By completing the Survey you will be deemed to have accepted these Prize Draw Terms and Conditions.
